ACCESSING ACMH SUPPORT
There are three key points of entry for people looking for referrals to ACMH's services
Care Management Services
ACMH provides outreach, engagement and care coordination serving hundreds of persons daily.
Supportive Housing
To make a referral to supportive housing, contact the Director of Intake and Transition Services.
Residential Crisis Support (Respite)
ACMH has two home-like settings that serve people needing a short-term stay during a psychiatric crisis or in transition from hospitalization.
SUPPORTIVE HOUSING
The Supportive Apartment Program offers supportive living apartments in Manhattan, Queens and the Bronx. With rent subsidies from the State of New York, residents are able to secure affordable permanent housing. Staff make regular visits to the residents in their homes and provide services according to an individualized support plan. An Emergency On-Call system provides clients’ access to mental health professionals around the clock.

- Approved NYC Supportive Housing Application (formerly the HRA 2010e form) with Level II and/or Community Care eligibility.
CARE MANAGEMENT SERVICES
ACMH Care Management Services assists adults to manage chronic mental health, substance use, and medical conditions. In partnership with Medicaid Health Homes and the NYC Dept of Health & Mental Hygiene (DOHMH), Care Management Services provide outreach, engagement and care coordination serving hundreds of persons daily.
Who is eligible for Care Management services?
If you are a Medicaid recipient seeking Care Management support, answer these questions to determine if you could use help from ACMH in connecting with needed healthcare. Contact us to determine eligibility.
- Do you have chronic or mental health conditions for which you need regular doctor’s care?
- Do you have a doctor you can see when you need to?
- How many times have you been in the emergency room or hospital in the past six months? Twelve months?
- Do you have a safe place to live?
- Do you have someone in your life to help you whenever you need help?
- Do you have difficulty keeping medical appointments?
What are some examples of how you assist people enrolled in Care Management Services?
ACMH support is tailored to the needs and goals of each individual that we serve. Your care coordinator will work with you to assess areas of your life that can be made better and work with you to develop a care plan to help you reach your goals and improve your overall health and wellbeing.
- With our help, Mr. X, a diabetic, now has a primary care provider, for the first time.
- Mrs. C was eligible for rental assistance benefits but didn’t know it – we helped her get those benefits.
- Mr. Z had trouble remembering appointments with his psychiatrist, so his Care Coordinator provided a OMNY Card and called him the day before, and on the morning of, each appointment.
- Ms. W was having trouble managing the symptoms of her depression and had never received mental health treatment. Her care coordinator helped to link her to treatment so that she could manage her symptoms and pursue employment.
- Miss T was homeless and pregnant. We helped her find housing and connect with primary and pre-natal care.
No, our clients determine their own treatment choices, including choice of providers.

Care Management Referral Process
Are you enrolled in Medicaid? If Yes, go to Step Three. If No, go to Step Two.
Contact the Medicaid Helpline to apply for Medicaid by calling
(800) 541-2831.
Once enrolled into Medicaid, go to Step Three.

ACMH Residential Crisis Support is a home-like setting for a short-term stay during a psychiatric crisis or in the transition from hospitalization. Available at two locations in Manhattan, Residential Crisis Support is staffed by a licensed mental health counselor and 24/7 support from peers with lived experience as consumers of mental health services.
